For this list, we’ll be looking at the most sickening characters and enemies in gaming, whether it be due to their appearance, behavior, or both. Since we are touching on story elements, a spoiler warning is in effect.

#20: The Huddle

“Inside” (2016)

“Inside” is one of the most stressful, yet hauntingly beautiful indie games to release in the 2010s. As a young boy, players must helplessly flee and hide from all sorts of hazards and foes. But by the end, they’ve turned into something to be hidden from. Referred to only as the Huddle, you join with this mass of writhing body parts and make your escape from a lab during the climax. Everything from its blob-like design to its unsettling movements to the groans from multiple inhabitants gives us the heebie jeebies. Thankfully, the camera always stays pretty far back.

#19: Anabella Rosfield

“Final Fantasy XVI” (2023)

We’ll admit it's solely Anabella’s personality that disgusts us. But boy oh boy, is it really disgusting. As a completely selfish Duchess, Anabella praised one of her sons and condemned the other. She betrayed Rosaria in favor of their enemies and forced Clive to be a slave to an army for years following the apparent death of Joshua. That would be enough to make us wish her the darkest of fates. But that’s only in the set-up. She also hunts down all who stand against her and turns her next son into a tyrant. Everything she does, and everyone she hurts, is all to gain power. And it all deliciously blows up in her face.

#18: The Aberdeens

“Red Dead Redemption 2” (2018)

On a physical level, the Aberdeens aren’t too much grosser than anyone else you’d find in “Red Dead Redemption 2.” But their actions certainly make it so. Siblings Bray and Tammy run their own pig farm and will warmly invite the player to share a meal with them. It becomes sickeningly clear throughout your interactions that they have an incestuous relationship. Every line from Bray will make your skin crawl. They also like to drug, rob, and kill people under the guise of hospitality, and you’ll find yourself their would-be victim should you accept their offer. But honestly, that’s far from the most offensive thing about them.

#17: The Master

“Fallout” (1997)

With nuclear fallout, you’re bound to come across more than a few characters with twisted designs. But the Master from the original “Fallout” is still hard to look at, even all these years later. While he may seem like one person, he’s actually the result of many people and mutants being melded together. The stretched out skin, the fact that he’s fused to a computer, the tendril-like appendage with a single eye…everything about him is unnerving. His end goal is to turn all other humans into mutants, bringing them within the fold of the Unity. Why anyone would want to look like this though is beyond us.

#16: Bo’ Rai Cho

“Mortal Kombat” Series (1992-)

‘Disgusting’ may be the norm in this fighting series, due to the insane amount of violence one can inflict on their enemies. But when it comes to actual gross behavior, none of the fighters top Bo’ Rai Cho. Introduced in “Deadly Alliance,” the drunken master can definitely lay the smack down. However, given his inebriated state, some of his attacks revolve around him vomiting on his enemies. Taking things one step further into the absurd, Bo’ Rai Cho can also fart on foes to stun or damage them. The grip he has on his bodily functions would be impressive if it wasn’t so vile.

#15: Gruntilda Winkybunion

“Banjo-Kazooie” Series (1998-2008)

This mean old hag serves as the bear and bird’s main villain. And she is one nasty piece of work, quite literally. She has all of the characteristics of your stereotypical, unpleasant witch, including green skin and a face full of warts. However, during the first game, you can learn a myriad of gross fun facts about her. These include putrid parrot puke being her favorite smell, salted slug toothpaste being her flavor of choice, and that her favorite sport is loogie flicking. Yuck. We’re not surprised she’s obsessed with turning beautiful, but that won’t change what’s on the inside.

#14: Infested Corpse

“Dark Souls III” (2016)

No offense to any maggot enthusiasts out there, but they’re some of the grossest creatures on the planet. So, when FromSoftware based an enemy’s physicality around them, it almost made us lose our lunch. Found in the Cathedral of the Deep’s graveyard and later in the Irithyll Dungeon, the Infested Corpse is far worse than your regular undead. Inside its body sits a humanoid-shaped mass of maggots that bursts from its chest. The squirming clump also sounds just as gross as you’d expect, undoubtedly bringing forth a groan from many players upon a first encounter. We commend the developers for keeping things fresh when it comes to undead enemies, but this is too far.

#13: Asphyxia

“Silent Hill: Homecoming” (2008)

Before “The Human Centipede” nauseated viewers, “Silent Hill” gave players a horrifyingly segmented boss to fight. After pulling one of your allies through a disturbingly fleshy hole in the wall, Asphyxia will crawl forth to mentally scar you. She’s composed of multiple bodies all stitched together, which is certainly an alarming sight to be sure. But her legs also end in hands, giving her an extremely disturbing gait as she shuffles around, swinging her strung-together torsos at you. She may only be the third boss, and the encounter might not last very long. But that’s something we’re actually grateful for.

#12: Boogerman

“Boogerman: A Pick and Flick Adventure” (1994)

In the 90s, developers tried to turn basically anything into a platforming mascot, no matter how ridiculous the premise. Enter: Boogerman. The platformer places you in the shoes of Snotty Ragsdale, who travels to the alternate dimension X-Crement as his alter ego. Armed with burps, farts that make him fly, and, of course, boogers, you’ll bring peace to the disgusting dimension. It clearly isn’t just the hero that’s designed to gross out; levels like Pus Palace and Mucous Mountain overflow with toilet humor. Seemingly designed to be every 6-year-old boy’s dream game, “Boogerman” is unlike any other game out there.

#11: Mara

“Megami Tensei” Series (1987-)

In the “Megami Tensei” series, and its spin-off “Persona,” players can catch and fight with figures from various religions and mythologies. Not all of them will be pleasing to look at. Mara is a demon from Buddhist mythology often associated with desire. The developers took that facet and warped it into one of the most shocking designs in the game. With a basis in male genitalia, Mara is seen with testicles in some appearances and riding a weird chariot in others. The deep green skin doesn’t help matters, but it isn’t as bad as the obvious veins or the inclusion of tentacles. If their goal was to revolt us, mission accomplished.

#10: Succubus

“DmC: Devil May Cry” (2013)

Not every fan was pleased with Ninja Theory’s would-be reboot of “Devil May Cry.” One thing is for certain, though: it retained the series’ unique enemy design. While many iterations of the mythological Succubus are of scantily clad vixens, the one here goes in the exact opposite direction. Looking like a mix between a demon, slug, and some kind of beetle, her repelling design pairs horridly with her tendency to vomit at Dante during their fight. On top of that, her connection to the in-game energy drink, Virility, makes our stomachs churn. It makes humans weaker thanks to the special ingredient that comes from her body. Ugh.

#9: Boomers

“Left 4 Dead” Series (2008-09)

Vomiting on players is a surefire way to make it onto a ‘Most Digusting’ list, and the Boomer more than deserves its place. Found in “Left 4 Dead” and its sequel, this special infected type has a bloated appearance that makes it look as if it’s about to burst. That’s gross enough, but it also has a seemingly endless supply of bile that it will hurl onto anyone near it, impairing their sight and attracting a horde of zombies. Even if you aren’t puked on, being too close when the Boomer goes down will just result in the same thing since it literally does burst upon death.

#8: Shamblers

“The Last of Us Part II” (2020)

With its focus on fungal infection, the undead of “The Last of Us” are repulsive to say the least. The first game’s Bloater was certainly hard on the eyes, but the sequel introduced a variant called the Shambler, which is just a bit worse. This enemy mutated differently due to its wet environment, featuring groups of pustules instead of fungal plating. These growths can spray spores to damage the player’s skin and, similar to the Boomer, it will explode upon defeat. Adding pus-filled cysts to the already gross Bloater was the right call to make us flee in terror. While The Rat King is the franchise’s peak in fear, Shamblers take the crown when it comes to disgust.

#7: Guardians

“Dead Space” Series (2008-)

Ask any Horror fan and they’ll tell you the Necromorphs of “Dead Space” are some of the most vicious and horrid-looking enemies in gaming. Despite the fact that it can’t move from its location, the Guardian is the most disturbingly disgusting in the group. Mutated from a human, these enemies stick to walls and attack all trespassers. With its chest cavity open, you can see all the organs on display. But if you get too close, it will lash out and make you regret it. The fully matured version can make weapons out of its body parts and spawn embryonic minions to chase after you.

#6: The Crones

“The Witcher 3: Wild Hunt” (2015)

We’ll likely never get over our first encounter with a Botchling, a monster born from the improper burial of a stillborn child. However, when it comes to disgusting appearances as well as behavior, nothing in “The Witcher” beats the Crones. Brewess, Weavess, and Whispess are three witch sisters that live in the Velen swamps. On top of a taste for eating children, they’re each more off-putting than the last. Weavess looks to have a hive nesting where her eyes should be, Whispess is never seen without her bag of body parts, and Brewess’s discolored, oversized limbs make her look monstrous, which she very much is.

#5: Marguerite Baker

“Resident Evil 7: Biohazard” (2017)

Speaking of bug nests in places they shouldn’t be, anyone who has encountered Marguerite Baker should not be surprised to find her here. Most members of the family undergo horrific transformations, but it’s the mother who grosses us out the most. Players must face her in the second house on the Baker estate. Her voice grows more demonic and her appendages elongate so that she can crawl around the walls and ceiling like a giant spider. That’s definitely cause for alarm, but not as much as the infestation in her crotch. She’ll happily unleash a swarm of bugs from it on poor Ethan, making us all gag in the process.

#4: Darlene Fleischermacher

“Dead Rising 3” (2013)

In the “Dead Rising” series, human bosses are referred to as Psychopaths, having snapped as a result of the outbreak. Out of all of them, Darlene gives even rotting corpses a run for their money in the disgust department. Players find the scooter-bound glutton hoarding a ton of food, which she refuses to share as she claims to need it all. Her introductory scene, where she farts, gorges, and vomits, can make any player cringe and sets the stage for the following fight. She’ll chase after the player on her scooter while also purging herself around the room; be careful not to slip. Even the way she dies is repugnant.

#3: Gurdy

“The Binding of Isaac” (2011)

As he flees from his mother’s wrath into the basement below, poor Isaac comes across all manner of twisted monsters. Despite the simplistic art style, many of them are pretty unpleasant to look at. The one who tops them all is the boss, Gurdy. Resembling a humanoid with a partially deformed face, Gurdy hides in a massive pile of entrails and other organs. It pops out only to attack, hurling blood orbs your way. Worse is when you face Mama Gurdy later, an evolution in horrid. This gigantic figure is fused to the wall, spraying vomit at the player in the form of various projectiles.

#2: The One Reborn

“Bloodborne” (2015)

Inspired by Lovecraftian mythos and leaning heavily into Horror themes, “Bloodborne” has some of the most intimidating bosses in gaming, but also some of the grossest. The One Reborn is the product of a ritual that fused many Yharnam citizens together. As such, it is entirely composed of writhing, rotting flesh. It’s quite the sight to behold, and hear as it oozes bodily fluid, whether on purpose or because you keep hacking away at it. Due to developer FromSoftware’s love of making every game as difficult as possible, it’s likely players got stuck here and had to suffer this monstrosity.

#1: The Great Mighty Poo

“Conker’s Bad Fur Day” (2001)

Well, it doesn’t get any grosser than something made entirely of feces. While “Conker’s Bad Fur Day” has a legacy for being completely and hilariously vulgar, the Great Mighty Poo is the peak of what it has to offer. Players can find the boss in a mountain of crap built by dung beetles. And still, seeing him rise from his pit comes as a complete shock, as does his surprisingly catchy theme. With literally every inch of the arena covered in poo, it’s hard to tell where he ends and the stage begins. Though you don’t have much time to consider it since he’ll be tossing turds at our adorable protagonist. Let’s just say it’s a very good thing we can’t smell video games.
