But the show does acknowledge this once when Elena gets her memories of being in love with him erased then as a result can't understand the idea that she ever was.

Major Spoilers - let's not count the time Sherlock drugged John for an experiment or the time Sherlock faked his death and didn't tell John that he was dead but let's get to the heart of the matter because of Sherlock John's wife was killed. That should end the friendship right there.
